You must be registered with us before using our Service. Please read the following information to help you with the registration process.
Transport is available to those living independently within the Shoalhaven Area. You must be able to get into and out of the vehicle with limited assistance. You may take a Carer with you to assist you, if necessary.
Have you registered with My Aged Care? My Aged Care is a central agency for older people who wish to use community services. If you are in the age groups above, please call My Aged Care on 1800 200 422 for assessment and referral. They can help you access more than just transport. Please remember to tell them you’d like to use Shoalhaven Community Transport.
Are you already receiving assistance through the Commonwealth Home Support program? We can help you with your transport needs – please call us for information.
Have you registered with the National Disability Insurance Scheme (NDIS)? Shoalhaven Community Transport is a registered NDIS provider. If you are younger than 65 (50/under for Aboriginal/Torres Strait Islanders) we can help you with your transport needs. We can offer transport estimates and a service agreement to help you manage your plan. Please call us for more information. If you don’t have an approved NDIS plan yet, remember to consider transport as part of your plan.
If you don’t fit into the categories above and have problems accessing public or private transport, please call us to see how we can help you. We will ask you some questions over the phone to determine your eligibility and give you information about our services.
You will also be asked for permission to provide some of your information to government departments in accordance with our Funding Body agreements (see Service User Privacy and Confidentiality Policy). By providing this information you will be assisting in the planning of future services and funding for Community groups in your area. You have the right to refuse your information being given to a government department.
After registration with our Service, we will send you a Service User Information Handbook which provides information about the services offered and your rights, and responsibilities, as our customer.
If you are assessed as being ineligible to use our service, we will suggest other services which may be more appropriate, and you will be advised of circumstances in which you could reapply.
We can also offer transport at a non-subsidised rate to those people who are in need of transport, e.g. those with insurance claims for injury, with approval from the insurance provider.
